Purpose of going to Japan is visiting friends and distant relatives (cousin's
or further relatives)In case the applicant is applying together with another individual who has a relative in Japan (up to the third degree) of relationship, follow the requirements for “visiting relative”.

All documents must be original unless otherwise stated.
1: PHILIPPINE PASSPORT
Broken passport is not accepted.
Passports must be signed and must have at least two(2) blank visa pages.
2: VISA APPLICATION FORM (※）
Application Form Size: A-4 size(21.0×29.7cm)
Application form should be filled in all items correctly.
If items is not applicable, please fill in [N/A].
Application form without date of application, signature and with blank item will not be accepted.
For the date of application, please fill in the date of submission to RELI TOURS AND TRAVEL (or to the embassy in case of direct application).
Embassy may deny the application that is filled in incorrectly or wrong information.
3: PHOTO
Size: 45×35mm, with white background.
Photo must be taken within 6 months.
Please write applicant's name and birthday on back side of the photo.
Photo must be pasted on the application form. -- Please put on glue each corners
4:BIRTH CERTIFICATE -ORIGINAL-
It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
If the birth certificate from Philippine Statistics Office(PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.
If the birth certification is "late registration",
Please submit in addition, a Baptismal Certificate and School Record(form 137) from high school or elementary and School Yearbook(if possible).
If there is no record in PSA,
Please submit a "Certificate of None-Record" from PSA together with a birth certificate from the Local Civil registrar.
5:MARRIAGE CERTIFICATE(if the applicant is married) -ORIGINAL-
It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
In case there is no record in PSA,
Please submit a "Certificate of None-Record" from PSA together with the Marriage certificate from the Local Civil Registrar.
Please attach AUTHORIZATION LETTER for each applicants*"4 / 5": AUTHORIZATION LETTER to JAPAN EMBASSY for PSA
AUTHORIZATION LETTER to Japan Embassy for PSA, Fill in and Attach with other required documents.
6:DAILY SCHEDULE IN JAPAN(TAIZAI NITTEIHYO) (※）

7:PROOF OF RELATIONSHIP
Documents such as pictures, phone calls detail, receipts, passport copies and explanation letter about the relationship with the applicant.
8:INVITATION LETTER FROM GUARANTOR IN JAPAN
For "Visa applicant`s name, please write in Alphabet as shown in passport.
As to the purpose and background of invitation.
Please fill in concrete activities your planing and background / circumstances of invitation in detail.
◆Vague description such as "Sightseeing", "Visiting Acquaintances" will not be accepted.
Please indicate specific contents of activities.
◆Depending on the reason for invitation, you need to submit supporting documents to prove it.
E.G. Medical Certificate, Copy of Maternal and Child health Handbook (Boshi-techo), Certificate of employment.
◎Please do not submit a copy of your Individual Number Card (My Number Card) or your Medical Insurance Card in Japan.
Japanese laws and regulations prohibit us (embassy and designated agency) to accept documents including the following numbers:
・INDIVIDUAL NUMBER in JAPAN ( It`s call "MY NUMBER")
・INSURE`S NUMBER of MEDICAL INSURANCE in JAPAN
・INSURED PERSON`S NUMBER of MEDICAL INSURANCE in JAPAN, etc.
For any reason, if you need to submit a copy of those documents,
Please conceal the numbered part (blot it out with black ink to be unreadable)
9:RESIDENCE CERTIFICATE(JUMINHYO) of the FRIEND,ACQUAINTANCE and GUARANTOR IN JAPAN
Juminhyo must be issued within three (3) months and described all family members without omission.
However, it should be without INDIVIDUAL NUMBER (KOJIN-BANGO) and RESIDENCE CERTIFICATE CODE (JUMINHYO-CODE).
For any reason, if you need to submit a Juminhyo with INDIVIDUAL NUMBER / RESIDENCE CERTIFICATE CODE, Please conceal the numbered part (blot it out with black ink to be unreadable)
◆If friend /acquaintance in Japan is a foreign resident,
please submit a copy of friend's Residence Card (both sides)
【If applicant will shoulder part or all of his/her travel expenses】
10:BANK CERTIFICATE -ORIGINAL-
Validity of Bank Certificate is three (3) months from the date of issue.
※Your own account / Company certificate is not accepted
11:APPLICANT'S INCOME TAX RETURN(Form 2316) clear photocopy(latest).
Applicant's Personal name
If applicant is business owner and submits the ITR without applicant's name, please attach the copy of Business Permit from city hall.
【If Guarantor in Japan will shoulder part or all of the applicant's travel expenses】
12:GUARANTEE LETTER
For "Visa Applicant`s Name", please write in Alphabet as shown in passport.
In case that multiple applicants have the same guarantor, please write the name of representative and "Other names, see attached list".
Then, fill in the list of Applicants and submit.
13:INCOME CERTIFICATE(Shotoku Shomei) from City Hall
TAX RETURN CERTIFICATE with Gross Income(Nouzei-shomeisho: form 2) from Tax office.
KAKUTEI-SHINKOKUSHO-HIKAE
BANK CERTIFICATE(Yokin Zandaka Shomeisho)
※"13" must be issued within three (3) months.
Submission of Withholding Slip(Gensen-Choshu-Shomeisho) in lieu of "13" is unacceptable.
Please note that tax certificates to be submitted must indicate gross income amount.
※Guarantor's income Certificate etc. is important in measuring the guarantor's capability to support the applicant. therefore, submission of more than one document(e.g. Income Certificate and Bank Certificate) is allowed.

14:PHOTOCOPY OF APPLICANT'S ALIEN CERTIFICATE OF REGISTRATION(ACR)
ISSUED BY THE PHILIPPINES GOVERNMENT.
Foreign residents with long-term or permanent status in the Philippines applying for a visa must submit in addition to the above requirements (except birth certificate and marriage certificate)
※Visa application of foreigners who are on short-term (temporary) visa in the Philippines can not be accepted.
